Philadelphia Pennsylvania under the care of the Philadelphia's Department of Behavioral Health and Intellectual diaAbility Services is shifting from the traditional care of individuals in communities to addressing the needs of the entire community. DBHIDS has been assisting individuals seeking thier care in our communities for over a decade. Their presence and resources support our communities to develop into safe and productive residental neighborhoods by filling in support gaps such as housing, mental and physical therapy, peer to peer mentoring, transportation and case managers to name a few.
Doing the work to support individuals DBHIDS realized that their efforts with their clients had barriers. Environmental barriers that also need to be addressed to help their clients live a happy producctive life. That's where population health comes into DBHIDS adoption of all residents of Philadelphia. Population Health is DBHIDS continuation to tranform our system of "CARE".
